Are you a skilled Mental Health Clinician? Do you have what it takes to play a key role in positioning our organization for future growth?
Working collaboratively with respective your team and the Patient Medical Home team, and reporting to the Director of Patient Medical Home (PMH), you are responsible for providing comprehensive quality mental health treatment to patients within the Calgary West Central Primary Care Network (CWC PCN). The MHC is a member of an interprofessional healthcare team utilizing collaboration, innovation and teamwork in the provision of primary care services that support the PMH within the CWC PCN.
The CWC PCN is an innovative, progressive organization that uses digital health care technology to collaborate with team members, physicians and in the provision of care. The use of improvement processes and data to inform decisions is fundamental to delivering patient care and working within an integrated team. Using quality improvement and data informing are important cultural principles at CWC PCN.
